Onboarding process
In response to a participant’s contact or referral, one of our Service Coordinators will approach them and identify what services they need, their expectations, goals, and other relevant information. We then provide participants with more information on alternative services that could help them meet their objectives and needs, which would get them closer to meeting those objectives.
As soon as the above details have been communicated and understood, a face-to-face meeting is scheduled with the participants’ stakeholders at a time that is convenient for them. Service coordinators will meet with participants to get to know them better, their dreams and aspirations, and help them tailor support that will help them reach their full potential.
It is at this stage that all the paperwork, including the service agreement and the care plan, is explained, completed, and signed. We understand things can change, so our team works hard at offering a flexible service that allows the participant to continue living their life their way. This is done with the peace of mind knowing we are readily available to hear about how we can improve.
Building a great team for the participants
Carers at Angels on Duty have a wide variety of skills and backgrounds that they bring to their work. These skills may be acquired through formal education, such as social work degrees and disability support certificates, or even through life or other work experiences. Moreover, we offer upskilling opportunities to our caregivers on a regular basis, as well as adequate training.
A person-centred, individualised service is provided by Angels on Duty, which takes into account the preferences of each participant, including (but not limited to) the type of carer they would like. In order to accomplish all types of goals in all areas of their personal journey and NDIS journey, the participant can choose their own carer with the right set of skills.
In order to match participants with the right carers, the AOD team will assess the nature and complexity of the participant’s supports. The process will also take into account the support worker’s personality and aim to match it with that of the participant. We are able to cater to the choice of support team regardless of whether a carer is bubbly and energetic or calm and composed.
Achieving Participants Goals
As is well known, the NDIS package focuses on building a plan for people with disabilities and identifying their needs. As a team, we are dedicated to helping people achieve their goals. As a way of demonstrating this, we aim to identify the participants’ circle of supports and their responsibilities in maximizing their outcomes. A reliable support circle is essential to growth.
In order to develop a care plan that highlights the participant’s objective, we identify the participant’s physical, social, or independent aspirations. This care plan focuses on areas that need improvement and ensures progress is monitored. Progress is progress, no matter how big or small it is.
As an example, we often speak of the need for an individual to become independent in their daily activities. In our role as a service provider, we establish a routine with the participant that includes tasks that are part of their everyday routine. After identifying their current level of independence, we collaborate with them to improve their capacity through coaching, inclusive engagement, active encouragement and guidance.
